Which toolbar has autofill

Need help with auto-fill the fields. I need help with a form that has multiple pages. I have some areas I are duplicated in my document. For example, I'm going to fname lname, address, telephone, etc How can I get these fields automatically on the other pages of the form? Is this possible? Any suggestions are welcome. If it's just a fill operation then make sure the fields have the same name.
